Our grant program is offered once a year.

The grant application is available from February 15, 2024 to April 30, 2024. Recipients and non-recipients will be notified by
July 15, 2024.

25 grants will be awarded yearly to families across the United States.

The MyGOAL Autism Grant is $1000.

The MyGOAL Autism Grant requires 3 documents to be uploaded at the time of submission, these documents are;
i. The Medical Health Assessment Questionnaire
ii. Grant Application Cover Letter
iii. Previous year’s Tax Return or proof of SSI Statement

The Medical Health Questionnaire is used to determine the applicant eligibility, medical information must be provided for the
applicant in order to be considered for the grant. It is essential for the grant committee to have the current health information, in
order to be able to assess the degree of severity of the individual’s health condition as a criteria for eligibility. This form should
be completed by the applicant’s physician and should be uploaded with the application for the MyGOAL grant.

This cover letter is used to describe your family’s situation and the need for the grant in order to be considered for the grant.
This form should be completed by the applicant’s primary care-giver and be uploaded with the application for the MyGOAL
grant. The following information should be included:
i. Number of dependents
ii. Number of dependents with Autism Spectrum Disorder
iii. Information about what current funding the grantee is receiving (i.e. medical, nutrition, personal, enrichment,
education and socialization needs)

It is understood that this grant will be used to benefit the overall quality of life for the individual(s) with Autism. Although
awarded to the primary caregiver, it is with the understanding that the grant will be used to benefit the individual(s) with Autism
Spectrum Disorders.

If the family was a recent (within 3 years of the date of this application) recipient of a grant award from MyGOAL, preference
will be given to a first-time application, based on other evaluation criteria.

If you receive a grant, you agree
i. to submit a brief video testament report of your experience and use of funds with photographs.
ii. to grant the MyGOAL Inc. the right to use your video testament for marketing, publicity, or any other purposes.
